To comply with local veterinary laws and regulations, an “informed consent sheet” request from your veterinarian asking for animal communication by Dr. Seabrook used as a “veterinary complementary modality” for your animal’s veterinary care must accompany your request.
This is the informed consent sheet to have your veterinarian complete when you request a veterinary complementary modality consult. It may be copied and pasted off the website, or Dr. Seabrook can forward you a copy via email.
